  • Telehealth visits when an in-person visit is not possible

  • Medication and prescription needs

  • Care coordination with pharmacies, home health, facilities, specialists, and other healthcare providers

  • Medical questions or changes in condition

  • Coordination of care following displacement or temporary relocation

  • Assistance navigating other healthcare needs that may arise during this difficult time.
