Clinical Care Navigator
Our Care Navigators play a vital role in coordinating patient care and ensuring smooth transitions across healthcare settings. This position involves managing patient enrollments, scheduling visits, communicating with facilities and families, and supporting providers with care coordination. Ideal candidates are highly organized, compassionate, and thrive in a fast-paced, team-oriented environment.
Requirements: Active MA or CNA license required
Advanced Practice Clinician (NP/PA)
Our Advanced Practice Clinicians—Nurse Practitioners and Physician Assistants—provide comprehensive, patient-centered care in post-acute and long-term care settings. APCs manage acute and chronic conditions, perform routine and urgent visits, collaborate with interdisciplinary teams, and guide care plans to improve patient outcomes. This role is ideal for clinicians who value autonomy, collaboration, and building meaningful relationships with patients and care teams.
Medical Director (physician)
The Medical Director provides clinical leadership and oversight across post-acute and long-term care facilities. This role focuses on ensuring quality of care, supporting and mentoring clinicians, collaborating with facility leadership, and guiding best practices in patient management. Medical Directors play a key role in shaping care delivery and improving outcomes for complex patient populations.
