Continuity of Care
Transitions of care are often one of the most vulnerable times in an older adult's life. We offer acute visits, post-ER/post-hospital visits, follow-up

Timeliness
In situations where immediate care is required, but an in-person visit is not feasible, we offer telehealth to bridge the gap and allow clinicians to assess the patient's condition promptly.

Insurance
PacWest assumes the role of the Primary Care Provider (PCP) and bills through insurance just as a community dwelling Primary Care Provider would.

Jason Heffernan, MD, MS.
Physician Owner
Dr. Jason Heffernan, originally from Spokane, Washington, commenced his medical journey as a hospitalist. It was during this tenure that his ardor for Hospice & Palliative care ignited. His path led him to deeply engage in skilled nursing rehabilitation and senior living communities, where he played a pivotal role in establishing the largest provider group at that time, catering to skilled nursing facilities, long term care facilities, adult family homes and adult living sectors.

Dr. Heffernan has contributed to the inception of national programs aimed at enhancing care for our elderly community. His vision is to assemble a dedicated team focused on bolstering provider support, to deliver unparalleled presence and care to patients and the facilities serving them.

Kristin Prey, DNP, MS.
Nurse Practitioner
Kristin is an Advanced Registered Nurse Practitioner (ARNP) with over 12 years of advanced practice nursing expertise, successfully serving in both acute inpatient and post-acute long-term care (PALTC) settings. She earned her Master of Science in Nursing degree in 2013, followed by a Doctor of Nursing Practice degree in 2021 from Gonzaga University.
Kristin began her ARNP career in the inpatient setting, where she developed a strong foundation in managing complex acute medical conditions under the mentorship of skilled physician colleagues. Over time, her focus shifted to long-term care—a setting where she found her passion and has spent the majority of her career. She specializes in caring for individuals with multifaceted medical and psychosocial needs across skilled nursing facilities, assisted living, adult family homes, and residential care communities. She particularly enjoys forming relationships and supporting individuals and their families through the complexities of chronic disease management. Throughout her career, Kristin has held a variety of leadership roles and consistently served as the clinical lead within her provider teams. She is deeply committed to professional development and mentorship, having supported numerous advanced practice students as a clinical preceptor.
Kristin is also the owner and operator of Osprey at Carnahan Adult Family Home in Spokane, Washington, which opened in August 2022. Her extensive experience in long-term care has given her a deep understanding of the needs of the aging population and the vital role families and caregivers play in providing high-quality support. This perspective also enables her to assist other adult family home owners by promoting regulatory compliance and fostering quality care practices.
Outside of her professional work, Kristin enjoys traveling and spending time with her husband and two young sons.

Jamie Finnerty, MSN.
Nurse Practitioner
I was born and raised in Spokane, Washington, obtained my Bachelors of Science in Nursing from Gonzaga University in 2008, and began practicing as a nurse on the neurology unit at Providence Sacred Heart Hospital in Spokane where I provided acute care for patients for 12 years. I decided to pursue an advanced practice degree in 2017 and obtained my Master of Science in Nursing from Gonzaga University in 2020 and board certification through the American Association of Nurse Practitioners. I joined Multicare Rockwood Clinic in November 2020 and have been practicing in an outpatient endocrinology clinic managing chronic conditions including type 1 and 2 diabetes, osteoporosis, thyroid and parathyroid disorders. When I am not caring for patients, I enjoy spending time with my husband and our four children.

Leigh Moseman, MSN.
Nurse Practitioner
Leigh Moseman is a board-certified Acute Care Nurse Practitioner earning a Bachelor of Science in Nursing (BSN) degree from the College of Saint Mary in 2004, followed by a Master of Science in Nursing (MSN), ARNP, and BC-ACNP degree from Creighton University in Omaha, Nebraska.
From recuperating open heart patients as an RN to opening Universal Cardiac Units in two large health systems in Omaha, Nebraska, Leigh has worked with a diverse population of patients. As an ARNP, Leigh was able to provide inpatient care at a Skilled Nursing facility before moving to Spokane, WA to work in the specialties of Cardiothoracic Surgery, Vascular Surgery, Thoracic Surgery, and Bariatric Surgery. In the following years, Leigh went on to provide services for mobile primary care, where she developed her expertise in working with diverse patient populations and a focus on providing psychiatric care to individuals at various stages of life, from acute to chronic. As a patient advocate across her wide range of professional expertise, Leigh maintains a strong patient focused care model. It is Leigh's privilege to practice her skills alongside her patients, their families, caregivers, and the staff at the facility.
She lives in Spokane, Washington with her youngest daughter and is constantly humbled by the outstanding individuals all three of her daughters have become. Spending as much time together as possible is a priority for them. Leigh's favorite hobby is gardening, whether she is growing tomatoes or flowers. Each summer, Leigh displays an impressive display of flowers in her neighborhood.

Julie Patrick, MSN
Nurse Practitioner
We are delighted to introduce Julie Patrick our esteemed Nurse Practitioner, who provides exceptional healthcare services to our community. With a robust background in nursing and advanced clinical training, Julie brings a wealth of knowledge and compassion to our practice.

Julie holds a master’s degree in nursing from Gonzaga University along with a specialized certification as a Nurse Practitioner. With over 35 years of experience in various healthcare settings, including emergency and critical care nursing at UCLA, Nurse Practitioner Faculty, Endocrinology, School Health, Public Health, and Palliative Care. Julie is well-equipped to handle a wide range of medical needs.

Julie practices with a patient-centered approach: Emphasizing personalized care plans tailored to each patient's unique needs and preferences. She believes in empowering patients with knowledge and resources to make informed decisions about their health.
Julie is passionate about fostering strong patient-provider relationships built on trust, empathy, and mutual respect. By taking the time to listen and understand each patient's concerns, Julie ensures a supportive and inclusive healthcare experience for all.

In her free time, Julie enjoys spending time with her family and various pets. She loves the outdoors and can be found camping, gardening, hiking, and water activities.

Brian Haberbush, BSN, NP-C
Nurse Practitioner
Brian Haberbush was born in San Diego, California but grew up just outside Spokane, Washington. He has been a Nurse Practitioner for 14 years and specializes in post acute rehabilitation and inpatient drug and alcohol treatment. He obtained his Nursing degree in 2006 and Nurse Practitioner degree in 2012 from Washington State University. His career has spanned assignments in Portland and in the Inland Empire of Spokane, including practicing medicine in the ICU, house calls, primary care, post hospital rehabilitation, and drug and alcohol treatment. His passion has been helping people find hope in sobriety and personal wellness. He strives to help people out of the trap of addiction and into a place where they can help themselves again. He believes communication is the key to good healthcare and endeavors to always keep an open line of communication with his clients. He currently resides in Spokane, Washington with his 13 year old son and 16 year old daughter. He especially enjoys taking his kids snowboarding, fishing, kayaking and traveling. In his next adventure, Brian is planning to explore Zion National Park in Utah and the Olympic National Forest in Washington State.

Polly Caudill
Administrative Assistant
Polly was born in Hamilton, Montana and moved to the Spokane, WA, Post Falls, ID area in 1991. She has three grown children and 8 grandchildren. She has been in the Medical Administrative field since 1999, and in the Mobile Primary Care field for the past 10 years. She has a passion to make sure the transition of care runs smoothly between the family members, guardians, patients, assisted living and adult family homes. She will go above and beyond to make sure this has been accomplished. Nothing makes her happier than to get a call from a Power of Attorney thanking her for the extra time she spent getting their loved one admitted into services, and their needs met. When not working, she spends most of her time spoiling her grandchildren, as they love to come spend the night with grandma. She also enjoys playing pool and spending time with family and friends.

Tom De Oro
Facility and Provider Relations
Tom de Oro is a Nursing Home Administrator (NHA) who brings 14 years of experience to our team. He has lived in our community for close to 10 years, working in both Spokane and Coeur d’Alene. Tom obtained his bachelor’s from Walla Walla University in Business Administration as well as his Master’s in Health and Policy Administration from Washington State University.
His strong passion for our seniors and knowledge of healthcare operations will help build the strong partnerships required for excellent clinical outcomes. Providing the highest level of service to both our providers and the facilities we serve is his driving mission. During his personal time, Tom loves to spend time with his wife and five kids on any of our neighboring lakes or outdoors.

Sydney Robinson
Billing Support Specialist
I was born and raised in rural southwest Virginia. I am a dedicated billing professional with a Bachelors of Science in Kinesiology from East Tennessee State University, where I graduated from in 2024. Since 2021, I have been actively working in the healthcare field, with a focus in medical billing. With plans to pursue higher medical education, I am passionate about advancing my knowledge and skills to better serve a wide healthcare community. In my personal time, I enjoy spending time with my friends and my dog, Artie

Marissa Bird, RN.
Chronic Care Management
I was raised in Sammamish, Washington and graduated from Pacific Lutheran University with my BSN in 1997. Concurrently, I commissioned as an officer in the Army Nurse Corps, where I began my career in nursing with military hospitals. I graduated with my MBA in Healthcare Administration in 2005. I transitioned to administrative roles, serving in many capacities while on active duty. Notably, I have been a career manager working for the Branch Chief of the Army Nurse Corps, an officer in Clinical Operations, culminating as the Chief Nurse of the 396th Combat Support Hospital. I retired in December 2020 as a Lieutenant Colonel after serving nearly 24 years. I settled in Spokane after meeting my spouse. We have two young boys who keep us busy with activity and laughter. I enjoy time with my family, camping, traveling, and doing most anything outdoors. I especially enjoy skiing, biking, and boating. I am an avid lover of music, animals, Broadway musicals and good food. I am also a sports enthusiast, particularly football – go Seahawks! After a long career with the military, I am looking forward to supporting the local Spokane community in my role with the PacWest team.

Sally S. Johnson
Nurse Practitioner
Sally Johnson graduated with a Bachelor of Science in Nursing from Montana State University in 2014, and has been a member of Sigma Theta Tau, National Honor Society of Nursing since 2013. She began her career in nursing working on a progressive cardiovascular unit in Great Falls, Montana. Sally has since served as the director of nursing and administrator where she helped start a home health agency in Coeur d’ Alene, Idaho, where she was instrumental in obtaining their initial accreditation from the Joint Commission. She then transitioned back to the hospital, where she worked on the cardiac transplant unit at Providence Sacred Heart in Spokane, Washington, while attending Baylor University in pursuit of her DNP-FNP degree. She graduated in 2024 from Baylor University. Sally is a more than five-time DAISY nominee and a DAISY Award Honoree. She currently lives in her hometown of Coeur d’Alene, Idaho. Sally is a loving wife and mother of four busy kids. She is an Idaho native and enjoys the beauty and activities her Northwest home has to offer.

Cindy Cohen
Nurse Practitioner
Palliative Care
With over 50 years of nursing experience, I am a dedicated Palliative Care Nurse Practitioner who continues to serve because of a deep passion for helping others. Originally from just outside Pittsburgh—lifelong Steelers and Penguins fan—I began my nursing journey right after high school and have pursued higher level education and training to support my role as a Nurse Practitioner specializing in palliative care. The true reward, however, lies in the personal connections I build, offering patients and their families the reassurance: “Let me walk this path with you” as we navigate the complexities of chronic illness together. Now living in Spokane with my husband of eight years, I’m proud mom to two accomplished daughters—one a college professor, the other a circus performer in Las Vegas—and grandma to several beloved cats. Life is good.

Amanda Pope
Care Navigator
Amanda Pope is a Certified Nursing Assistant with over 15 years of experience in healthcare, specializing in direct patient care within long-term care facilities and rehabilitation centers. She has worked with a diverse range of patient populations and has particular expertise in supporting individuals with dementia and Alzheimer’s disease.
Originally from a large family of 13 children, Amanda has spent most of her years in the Pacific Northwest, where she developed a deep appreciation for nature.
Outside of work, she enjoys hiking, exploring the outdoors, reading, and seeking out new experiences that reflect her curiosity and adventurous spirit.

Naomi Rosen, RN.
Chronic Care Management
Naomi has been an RN for over 20 years, with most of her experience in
various settings in Los Angeles before relocating to Spokane in 2021. She
graduated from nursing school in Bend, Oregon, and then received her
BSN in 2014. While in Spokane, she has focused on mobile primary care,
understanding the uniqueness of this specialty.
Naomi is passionate about being an advocate for her patients and
ensuring they achieve the best possible outcomes in their health journey.
She enjoys interacting with a diverse patient population and finding
solutions to problems within each individual’s wishes and or
circumstances.
Naomi enjoys watching baseball (especially the LA Dodgers), spending
time with her 4-year-old whoodle, Koko, and cooking with her chef and
baker husband.

Madeline Hill
Nurse Practitioner
Madeline D. Hill, ARNP, was born in Spokane, WA and is a graduate of Whitworth University, also located in Spokane. She received her Bachelors in Science of Nursing. After working for a couple years as a nurse, Madeline attended Gonzaga University, in Spokane, and received her Master's in Science of Nursing, Family Nurse Practitioner.
As an undergraduate nurse, Madeline started her career in Labor and Delivery. However, she soon discovered her interest in Geriatric medicine. During her masters degree program, she worked as a nurse in a skilled nursing facility and assisted living. Upon graduation, she began her practice as a nurse practitioner in Post Acute/Geriatric medicine. Madeline’s passion in this field stems from the ability to form trusting relationships with her patients, and work hand in hand with them during their health journey. She has had the privilege of providing care in various skilled nursing and long term care facilities in Spokane over the past 13 years.
During her free time, Madeline enjoys spending time with her family, which includes her husband and two children. They especially enjoy outside activities, such as camping, skiing and hiking.

Ashley Smith
Nurse Practitioner
Born and raised in Spokane, Washington, Ashley Smith is a board-certified Family Nurse Practitioner with a passion for delivering compassionate, comprehensive care. She earned her Bachelor of Science in Nursing from Gonzaga University in 2013 and began her career as a Registered Nurse at Sacred Heart Hospital, where she provided acute care to complex nephrology patients, including those requiring dialysis.
Ashley went on to serve in the advanced care unit at Holy Family Hospital, specializing in cardiac and telemetry care for acutely ill patients. She later transitioned into a triage RN and care management role in a family practice clinic while pursuing her Master of Science in Nursing, which she completed in 2021, also from Gonzaga University.
Over the past three years, Ashley has worked in family medicine, providing primary care across the lifespan. Most recently, she has shifted her focus to community care, working closely with geriatric patients to support their unique healthcare needs.
Outside of work, Ashley enjoys an active life with her husband, their two young daughters, and three dogs. As a family, they love to travel, run, bike, and spend time outdoors whenever possible.

Jordan Van Gelder
Care Navigator
I am a patient care navigator with a passion for making a difference in the community. I have dedicated the last few years of my life to helping local shelter animals find loving homes and have found my way here at Pacwest where I plan on bringing the same level of compassion and care.
Born and raised in Spokane, it has always been important to me to help make an impact locally and I am looking forward to being able to do that.
In my free time I enjoy spending time outdoors with my dogs, painting and reading.

Jason Van Gelder
Nurse Practitioner
Psychiatry
I’ve been a nurse for over 22 years, with most of my experience in psychiatric care. For 17 of those years, I worked at Tamarack Center, an inpatient psychiatric facility for adolescents in Spokane. It was there that I developed a deep passion for mental health and witnessed firsthand the growing shortage of psychiatric providers in our community. That need inspired me to go back to school and serve in a new way.
About five years ago, I returned to graduate school—starting at Gonzaga and later transferring to Walden University during the pandemic, where I completed my master’s degree in nursing with a focus on psychiatry in 2024. My years as a psych RN have been invaluable and continue to shape the way I practice today as a Psychiatric Mental Health Nurse Practitioner.
Currently, I’m proud to be part of the team at PacWest, where I’ve been working for the past five months. I truly appreciate the collaborative spirit and shared commitment to patient care that everyone brings to the table.
Outside of work, I love spending time outdoors—camping, fishing, hunting, hiking with my dog, or playing a round of golf. But more than anything, my greatest joy is being a father. My son is my proudest accomplishment, and time with him is what matters most.

Shevonne Ramos
Care Navigator
I’m a patient care navigator at PacWest Healthcare. I always had the passion to help and serve my local community. Previously worked at Holy Family hospital as EKG technician for few years before transitioning into the care navigator role. I have experience when it comes to providing patient care and backbone support for providers.
I graduated from Gonzaga University with Social Science major. In the future, I hope to strive into receiving my master’s degree and becoming a mental health counselor.
During my free time, I enjoy trying new restaurants, road trips and have horror movie marathons.
